Our offices are located on the  Baptist Rehabilitation Hospital Germantown campus at 2120 Exeter Road, Suite 130. We are open for appointments Monday through Friday 8AM to 5PM.

Call us and we can fax you a copy of our registration information forms in preparation of your visit. Bringing them in filled out will expedite your appointment.

Please be aware that we are unable to fill prescriptions after hours or on the weekends, leaving a trip to the emergency department as you only option if you run out of medication between Friday evening and Monday morning. Plan ahead!
